Patient Admission Guide

Your Responsibilities as A Patient

During admission, you will be interviewed by our staff. The information that you will provide
shall become a hospital record. An admission kit will be provided for the the patient’s toiletries and
other essentials.


As a patient at Urdaneta Sacred Heart Hospital, Inc., you and your representative are expected
to:

 

  • Provide complete and accurate information about your health, including presentcondition, past illnesses and hospitalizations, medications, natural products, and vitamins,and any other matter that pertains to your health.

  • Provide complete and accurate information, including your full name, address, hometelephone number, date of birth, insurance carrier, and employer when necessary.

  • Provide your doctor or the Hospital with a copy of your advance directive if you have oneand want it to apply during your admission.

  • Ask questions when you do not understand what your doctor or other health care team member will tell you about your diagnosis and treatment. You should inform your doctor if you anticipate problems following the prescribed treatment. Inform your doctor if you are considering alternative therapies.

  • Ask your doctor or nurse what to expect regarding pain and pain management plan. You should tell your doctor or nurse about any worries you have about taking pain medication.

  • Leave valuables at home and bring only the items you need during your hospital stay. The Hospital is not responsible for their loss.

  • Abide by all hospital rules and regulations.

  • Comply with the NO SMOKING policy.

  • Comply with Visiting Hours (10 am to 12nn & 3 pm to 9 pm) and Policies to ensure the rights and comfort of other patients. Be considerate of noise levels, privacy, and safety.

  • Treat hospital staff, other patients, and visitors with courtesy and respect.

  • Provide complete and accurate information for insurance claims and work with the

  • Hospital’s Credit and Collection Staff to ensure proper payment settlement.

  • Know your health insurance coverage and related policies, including the Hospital and doctors covered by our insurance provider and its policies about required pre-approvals, covered services, and admissions.

  • Ask questions of your insurance company or the Hospital’s Billing Services if there is a the financial issue that you do not understand.

  • Watchers are not allowed to sleep/sit on vacant beds; the bed occupancy will be charged to the patient’s hospital bill.

  • Firearms, alcoholic beverages, or any intoxicated drinks are not allowed inside the Hospital.

  • Any loss or damage to hospital property shall be charged to your account.

  • Children 7 years old and below are not allowed to visit relatives and friends confined in the Hospital for your children’s and patients’ safety.

  • Mass is conducted every 1st Friday of the month, 4:00 pm at the Hospital Chapel, 2nd floor near Nurse Station 2C.

  • Should you have any queries or clarifications regarding your bill, please feel free to see our Billing Clerk at the Business Office, 2nd Floor, Room No: 203.

  • Cut-off time for patients’ meals will be as follows.

    •  6:00 AM

    •  11:00 AM

    • 5:00 PM

  •  Discharge hours will be as follows, except for emergency cases.

    •  8:00 AM – 12 NN

    •  1:00 PM – 5:00 PM
